JOB TITLE: COMMUNITY PSYCHIATRIC NURSE/CARE COORDINATOR
MAIN DUTIES: CASE LOAD MANAGEMENT:
Managing a case load up to 25- 30 clients. Covering Team Duty emergencies Covering depot clinic, Administration of medication, monitoring of positive and negative effects of medication, mental state assessment and referral to Approved Mental Health Professional (AMHP) for Mental Health Act assessment or referral to Crisis Resolution Home Treatment Team (HTT) for support if required, administering depot injection, managing clients on CTO, Formulating health care plan, implementing and evaluating the care plan holistically within evidence based models of care, work jointly with clients, colleagues, other professionals, relevant agencies, families and friends of clients, Carer’s assessment, Risk assessment & management and prompt action on any high risk identified, writing reports such as Social Circumstance reports, Tribunal Reports, Care Act assessment, Panel Presentation on behalf of client for Care Package or Direct Payment application for personal budget, writing letters to other agencies on client’s behalf, working in partnership with other health and social care agencies to implement any care package allocated, arranging CPA review meetings & CPA reports, arranging & attending medical reviews, attending professional meetings, attending Child Protection Conference and Child In Needs meeting, assisting clients to complete benefits forms, encouraging clients to engage in community activities for social integrations, attending MDT meetings, maintaining accurate and up to date documentation, maintain client’s privacy and confidentiality, Work in accordance with Trust policy and maintain Professional conducts etc.
Receives referrals from other Agencies, triage referrals to establish treatment options, managing walk-in clinic, advise and sign-post to required service.
